1st reaction is their server is only a two core system and the game has to load all the custom skins from every pilot and it ran out of horsepower to load everything it should have. vG said as soon as Loopy fired his guns he popped into view which still leads me to support this idea.
We had 38-40 people when I was watching the loads during the mission. If half or more had their squad skins on then the server had to keep up with quite a number of skins. The way the game treats it is that it always displays the regular default skin and then paints the custom skin on after that in about a second later. Also the game can not skin more than 5 planes around each pilot, you can have 8 planes around you and as you look around it will pick 5 (usually the closest in view) and displays those.
I think it got in between the default skin and the BH skin and got hung up before the BH skin loaded. They can reduce this from happening by locking skins but I know they want squads to have their paints so its a trade off. if someone says they can't see you then you squeeze off a round real fast and it makes the game pay attention to your plane and "should" fix the issue as long as the server has the horsepower to do the work.
The skins for ROF are pretty big files so it places a good load on the server so I think that's what we ran into here.
